Field Self Survey audit to assess machine guarding and control compliance and develop a management plan
Using the ReiTech Field Self Survey process, a knowledgeable instructor or school district safety
representative performs a comprehensive inventory/audit of each piece of powered equipment located in the
classrooms and/or maintenance rooms. The forms that you may download below are machine type specific and
guide you through your survey, question by question.
Once the forms are completed, the person doing the survey may also take a few pictures of each machine
surveyed to complete the inventory process. The Field Self Survey process breaks down into these simple

- Print out the forms in sufficient quantity to perform the surveys of your machines
- Using one form per machine, do the survey
- Take several pictures of each machine to document the survey
- Send the pictures, completed survey forms and school cover pages to us
